Queen's Domain Endings
Four ending achievements are listed. Community reports describe the first as the default outcome, the second as requiring attacks with reforged Akunra, and the third as requiring Okbujo's Judgement. One report adds a 69-skull ritual for the fourth. Having the right sword in your inventory did not suffice for one player; the exact damage threshold remains a single player's estimate.

Reported branches and the remaining disagreement
In the Ending 2 discussion, Selph and thoughtlostme both advise using the relevant sword in the fight, rather than merely carrying it. bb suggests dealing 25–50 percent of the damage with it. That range is one player's estimate and is not a verified numeric threshold. Scumdrop’s original report of getting Ending 1 with the upgraded sword remains relevant.
The Herald discussion describes the Memento Mirror, eight NPC mementos and forging Okbujo's Judgement. A separate book discussion also describes eight items and the mirror for Endings 3 and 4. The 69-skull ritual for Ending 4 appears in bb’s branch summary and thoughtlostme’s inventory note; the exact ritual dialogue is not reproduced by these sources.

FAQ
How many endings are there?
Four achievement names. That count is the achievement list, not a developer interview.
Does the true sword guarantee ending 2?
No. At least one report got Ending 1 while holding Akunra. I am not upgrading the sword into a switch.
