No weapon named Moonlight Sword is documented in the sources below. A community list headed 77/77 has 67 visible main-weapon names and does not list it, and a Steam thread asks the question outright. The nearest match on a single password guide is Starlight Blade, from the grimoire password SAOMATIC.

Why the name keeps coming up
King's Field and several later FromSoftware games put a moonlight sword in the player's hands, so the search is a reasonable guess. Queen's Domain borrows the slow first-person pace and then stops. I am not going to invent a hidden blade to satisfy the guess. The thread title on the Steam hub is the whole public argument: players are asking whether it exists. The endgame checklist that tries to name every main weapon does not have a row for it.
With's Grimoire is a password toy you pick up at a grim mark on the starting beach. One Steam guide tabulates the words and the rewards. Starlight Blade is the row that guide offers as the moonlight-shaped stand-in: a slow sword, a magic fire wave, and those two small percentages. A second source that repeats the same password and the same stats was not in the pages I kept. The password stays labeled as one source. What to use instead for a light attack
If the thing you wanted was a glowing swing, the Azure Torch throws an azure fireball and lights the beacons. If the thing you wanted was a long-term sword, Pandora's Blade is the level-100 project in the first dungeon. Neither one is a moonlight greatsword, and I would rather say that than rename them.

Will a patch add it?
The official Steam patch notes through 1.01.03 do not mention a new sword by this name. I have no roadmap that promises one.
Where do I type the password?
In With's Grimoire, the item from the beach. The guide that lists the word also lists the item. I am not repeating a button prompt I have not seen quoted twice.
